Recently, the media has been reporting on a study claiming that high doses of vitamin E might be associated with an increase in mortality. This meta-analysis pooled the data from nineteen previous studies and found that doses of vitamin E at or below 150 IU per day tended to lower mortality statistics while doses higher than 150 IU per day tended to raise mortality statistics.

While an indirect study like this may raise interesting questions, many researchers believe it cannot provide scientific proof of anything. That’s because, as a meta-analysis, it is based on statistics derived from the data from other studies, all of which were performed in different ways, using population groups with very different characteristics. For example, some of the studies cited used natural vitamin E, while others used synthetic vitamin E. Some of the studies involved people with cardiovascular disease, while others involved people with cataracts. Some of the studies combined vitamin E with other vitamins, while others used it alone. Because of the diverse characteristics and methodologies of all these studies, any analysis based on pooling of their data is considered scientifically imprecise.

Several other problems exist with this study which cannot allow us to draw any conclusions about vitamin E. For example, as diverse as the study populations were in the meta-analysis, most had one characteristic in common: they were chronically ill. Very high doses of vitamin E (up to 2000 IU per day in some of the studies), or any nutrient for that matter, must be used with caution in persons with serious diseases – i.e. only after a thorough review of the person’s medical history, current condition, use of medications and so forth. The fact that high doses of vitamin E were associated with a slight increase in mortality in a sick population provides no evidence that vitamin E is harmful to normal, healthy people.

Perhaps the most profound flaw in the meta-analysis, however, is that all the studies it cites use vitamin E in the isolated “alpha-tocopherol” form. Vitamin E is actually a family of nutrients consisting of four tocopherols (alpha, beta, gamma, and delta) and four tocotrienols (again, alpha, beta, gamma, and delta). Vitamin E, in its food form, always exists as some combination of these eight elements. In most nutritional supplements, however, vitamin E, whether natural or synthetic, is provided only as alpha-tocopherol.

Vitamin E should always be provided as a complex – in other words, as alpha-tocopherol plus mixed tocopherols. Ideally, the tocotrienols should also be provided. The authors of the meta-analysis themselves acknowledge that isolated alpha-tocopherol has been shown to displace other members of the vitamin E family, like gamma-tocopherol. They offer this as a possible explanation of their negative findings on vitamin E.
